Renal dosing, restrictions and evidence limits

For severe renal impairment (CrCl <30 mL/min, Cockcroft-Gault using actual body weight), the source gives once-daily doses of 14 mg for differentiated thyroid cancer, 10 mg for renal cell carcinoma, and 10 mg for endometrial carcinoma. No renal dose adjustment is recommended in mild or moderate impairment. There is no recommended dose for hepatocellular carcinoma with severe impairment. Do not transfer one indication's dose to another or invent a dialysis schedule.

No renal adjustment is recommended for CrCl 30 to 89 mL/min. For severe renal impairment, defined in the dose subsection as CrCl below 30 mL/min, the recommended daily dose is 14 mg for differentiated thyroid cancer, 10 mg for renal cell carcinoma, and 10 mg for endometrial carcinoma. No recommended severe-impairment dose is provided for hepatocellular carcinoma. End-stage renal disease has not been studied. Renal failure or impairment during treatment also requires severity-based withholding or discontinuation in the full label.
