Renal dosing, restrictions and evidence limits
Use actual body weight. Begin IV infusion at 0.5 ng/kg/min; increase by 0.5 ng/kg/min every 30 minutes as tolerated, to at most 2 ng/kg/min. Infuse for six hours daily for up to eight consecutive days; the full label specifies subsequent-day titration.
At eGFR below 30 mL/min, retain the usual starting and titration approach; if 0.5 ng/kg/min is not tolerated, reduce to 0.25 ng/kg/min. Consider administration after hemodialysis or beginning dialysis at least one hour after the infusion finishes. Dialysis effects on exposure remain unassessed. Correct hypotension first and monitor vital signs.
