Renal dosing, restrictions and evidence limits

Initial adult dose: CrCl >=60 mL/min, 15 mg/kg once daily; CrCl <60 or ESRD, 7.5 mg/kg once daily, a 50% reduction. Use actual or ideal body weight, whichever is lower. On dialysis days, give the dose after hemodialysis. The source states that the renal study used 24-hour urine creatinine clearance.

Monitor blood counts closely, with the source recommending every two weeks; subsequent escalation and toxicity interruptions require the full hematology protocol. Hydroxyurea can interfere with urea/uric acid assays and some continuous glucose sensors. Do not apply the adult starting dose automatically to pediatric sickle cell treatment or other hydroxyurea products.
