RECOMBIVAX HB dialysis formulation and ENGERIX-B both use a high-dose adult approach, but their concentrations, injection volumes and series schedules differ. CDC directs readers to each product insert.

Before applying: Verify actual vaccine, formulation, prior doses and anti-HBs follow-up. Do not convert one product's volume or three-dose schedule into the other product's regimen.

Renal dosing, restrictions and evidence limits

For adult predialysis or dialysis patients, the dialysis formulation contains 40 mcg in 1 mL. Give three IM doses at months 0, 1 and 6. This is not the standard 10-mcg adult formulation. The dialysis formulation is approved for adults at least 18 years old; pediatric dialysis safety/effectiveness have not been established.

Consider booster vaccination or revaccination if anti-HBs is below 10 mIU/mL one to two months after dose three. Assess antibody levels annually and give a booster if they fall below 10 mIU/mL. Apply the full vaccination history and current immunization policy, not a renal drug-reduction algorithm.

CDC advises high-dose (40 mcg) ENGERIX-B or RECOMBIVAX HB for adults receiving hemodialysis who need hepatitis B vaccination and directs clinicians to the individual package insert for the complete schedule. This supports the high-dose approach but does not make the products' volumes or dose counts interchangeable.
