Renal dosing, restrictions and evidence limits
Use conservative initial and maintenance dosing in impaired renal function. Renal insufficiency can increase glyburide exposure and severe hypoglycemia risk; older, debilitated or malnourished patients are particularly vulnerable. Assess renal function during dose selection and monitor for prolonged or recurrent hypoglycemia. The source does not supply a numeric CrCl reduction. Do not interpret conservative dosing as proof of suitability in every degree of impairment or transfer milligram doses directly to micronized GLYNASE.
