1 additional source qualifications - review before use
An IR renal schedule is not approval to use an XR capsule

The immediate-release source supplies a severe-renal instruction but leaves a separate PK evidence gap in other renal ranges.

Before applying: Retain formulation and the source limitations; do not infer an intermediate regimen.

Also check: An IR renal schedule is not approval to use an XR capsule

Creatinine clearance (CrCl), mL/min; no equation inferred where the source does not name one.

RX mirror: this is not independent corroboration of its underlying label. Check dated-label and formulation limitations.

Renal dosing, restrictions and evidence limits

At CrCl <30 mL/min, the immediate-release SANCTURA recommendation is 20 mg once daily at bedtime, rather than the usual 20 mg twice daily. Give on an empty stomach or at least one hour before meals. The source reports that PK in CrCl 30-80 has not been studied and notes greater adverse-reaction risk with renal impairment. The severe-renal instruction is not an extended-release capsule regimen.
