Renal dosing, restrictions and evidence limits
Renal disease and hypoalbuminemia increase the unbound phenytoin fraction. Monitoring should use unbound concentrations. Conversion from fosphenytoin to phenytoin may increase without a corresponding increase in phenytoin clearance, increasing toxicity risk. Consider the phosphate load when phosphate restriction is required in severe impairment. This card supplies no automatic renal dose reduction or infusion-rate substitution.
