Usual Dosing (Adults)
Herpes zoster: 500 mg orally every 8 hours for 7 days. Recurrent genital herpes: 125 mg twice daily for 5 days. Primary genital herpes: 250 mg three times daily for 5 to 10 days. Chronic genital-herpes suppression: 250 mg twice daily.
Renal Dosing
CrCl >60 mL/min: no change. CrCl 40 to 59: zoster 500 mg every 12 hours, other regimens unchanged. CrCl 20 to 39: zoster 500 mg every 24 hours; recurrent genital herpes 125 mg every 24 hours; suppression 125 mg every 12 hours. CrCl <20: zoster 250 mg every 24 hours; recurrent genital herpes and suppression 125 mg every 24 hours.
Hemodialysis
Recurrent genital herpes or suppression: 125 mg after each dialysis. Zoster or genital herpes in an HIV patient: 250 mg after each dialysis.
