Renal dosing, restrictions and evidence limits

The dated label names MDRD eGFR but prints mL/min without an indexing denominator. Its adult table gives combined doses: 60-129, 2.5 g every 8 hours over 2 hours; >=130, the same dose/interval over 4 hours; 30-59, 1.25 g every 8 hours; 15-29, 1.25 g every 12 hours; below 15 or intermittent HD, first-day 1.25 g loading dose then 0.625 g every 24 hours. All reduced regimens infuse over 2 hours; give after HD.
