Renal dosing, restrictions and evidence limits

No dose adjustment is required for mild or moderate renal impairment with CrCl >=30. Below 30, use is not recommended because this population has not been evaluated. Minimal urinary elimination does not override the severe-renal recommendation. Indication, titration and CYP3A4-interaction adjustments must be considered separately.

Renal dosing, restrictions and evidence limits

No dosage adjustment is required at CrCl at least 30 mL/min. Use below 30 mL/min is not recommended because that population was not evaluated. Because parent drug and active metabolites have long half-lives, monitor adverse effects and response for several weeks after starting or changing a dose. A renal restriction does not replace indication-specific titration.
