Renal-adjusted loading and maintenance doses

Use Cockcroft-Gault CrCl in mL/min. All infusions last 3 hours.

  • >50: load 2.67 g, then 2 g every 6 hours.
  • >30 to 50: load 2.67 g, then 1 g every 6 hours.
  • >15 to 30: load 1.8 g, then 0.9 g every 8 hours.
  • <=15, including HD: load 1.33 g, then 0.9 g every 12 hours.

Timing and limits

Maintenance starts at the next dosing interval after the single load; intervals run from infusion start to infusion start. Give after HD on dialysis days and monitor changing renal function closely. The labeled course is 5-14 days with concurrent metronidazole. Approval rests on limited clinical safety/efficacy data.
