On dialysis days, administer after dialysis. Apply these doses within the indicated treatment cycle, not an invented continuous schedule.
For MM combination therapy at CrCl 30-60, the label permits considering 15 mg after two cycles if 10 mg was tolerated without dose-limiting toxicity. Subsequent changes depend on indication and tolerability.
