Renal dosing, restrictions and evidence limits
Use is not recommended at CrCl <30 mL/min or in ESRD. Mild impairment (CrCl 50 to 80) needs no adjustment. In moderate impairment (30 to 50), use caution when initiating or escalating from 5 to 10 mcg; use caution in renal transplant recipients.
