Dosing / renal protocol and limits

Gentamicin, adult Hartford method: 7 mg/kg IV; CrCl >=60 every 24 hours, 40-59 every 36 hours, 30-39 every 48 hours. Not recommended below 30, on HD or CRRT. Check a level 8-12 hours after dose one and use the actual nomogram. Exclude pregnancy, major burns and significant ascites; use adjusted weight for obesity. Initial schedule only.
