Historical renal / replacement-therapy guidance

The book retains usual dosing at 30-50 mL/min and adds caution at 10-30 and <10. PD/HD/high-flux follow the <10 band; continuous therapy follows 10-30; removal is considered unlikely. It explicitly contrasts older UK caution below 30 with U.S. no-reduction advice. Its 0.75-1.5 mg weekly range predates current U.S. higher strengths and must not define the present maximum. Monitor dehydration-associated kidney injury.
