BLUJEPA: moderate impairment is not always sufficient for eligibility
The newer label includes gonorrhea-specific combined exposure risks not covered by a simple uUTI renal cutoff. No adjustment within a renal band does not override indication-specific QTc avoidance conditions.
What to reconcile: Confirm indication, weight, interacting medicines and hepatic function; do not invent a reduced gonorrhea regimen to bypass an avoidance condition.

Renal impairment comments
No renal adjustment is required at eGFR 60-89 or 30-59 mL/min. Avoid use below 30 mL/min, including dialysis, because increased exposure can increase QT-prolongation risk.

Indication-dependent caution
For the gonorrhea indication, avoid use in moderate renal impairment when other factors that increase gepotidacin exposure are present. This renal comment does not supply an indication-specific antibiotic course.
