Renal dosing, restrictions and evidence limits

The February 2026 label needs no dose adjustment at eGFR 30-89 but avoids use below 30, including dialysis. For gonorrhea, avoid moderate CYP3A4 inhibitors and avoid the combination of two or more exposure factors: weight 45-60 kg, eGFR 30-59, or Child-Pugh B. Strong CYP3A4 inhibitors and severe hepatic impairment are additional avoidance settings. The higher exposure/QTc risk is not addressed by inventing a reduced course. This supplement prevents an older uUTI-focused local summary from being generalized to gonorrhea.

Renal impairment comments

No renal adjustment is required at eGFR 60-89 or 30-59 mL/min. Avoid use below 30 mL/min, including dialysis, because increased exposure can increase QT-prolongation risk.

No dose adjustment is required at eGFR 30-89, but avoid BLUJEPA below 30, including dialysis, because increased exposure can prolong QTc. For uncomplicated urogenital gonorrhea, additional restrictions apply: avoid with moderate CYP3A4 inhibitors or when two or more exposure factors coexist (weight 45-60 kg, eGFR 30-59, Child-Pugh B). Thus, the isolated phrase no adjustment at moderate impairment does not establish eligibility for that indication. Strong CYP3A4 inhibitors and other label QTc precautions also remain relevant.
