BLUJEPA: moderate impairment is not always sufficient for eligibility
The newer label includes gonorrhea-specific combined exposure risks not covered by a simple uUTI renal cutoff. No adjustment within a renal band does not override indication-specific QTc avoidance conditions.
What to reconcile: Confirm indication, weight, interacting medicines and hepatic function; do not invent a reduced gonorrhea regimen to bypass an avoidance condition.

Renal dosing, restrictions and evidence limits
The February 2026 label needs no dose adjustment at eGFR 30-89 but avoids use below 30, including dialysis. For gonorrhea, avoid moderate CYP3A4 inhibitors and avoid the combination of two or more exposure factors: weight 45-60 kg, eGFR 30-59, or Child-Pugh B. Strong CYP3A4 inhibitors and severe hepatic impairment are additional avoidance settings. The higher exposure/QTc risk is not addressed by inventing a reduced course. This supplement prevents an older uUTI-focused local summary from being generalized to gonorrhea.
