The retrieved 2026 official label retains both a renal dosage table and an avoid-use warning for chronic renal failure. This is an unresolved within-label applicability tension, not evidence that either passage can be discarded.

Renal dosing, restrictions and evidence limits
The renal table gives two 0.18-mg tablets four times daily (1.44 mg/day) at eGFR 30-89.9 mL/min/1.73 m2, and one tablet four times daily (0.72 mg/day) below 30, including ESRD or dialysis. Timing need not follow dialysis. However, the same label warns to avoid use in chronic renal failure. Do not treat the table as automatic eligibility. Monitor vital signs, bradycardia and orthostasis; renal impairment also warrants ECG assessment. Taper rather than stop abruptly.
