Dosing / renal protocol and limits

Adult type 2 diabetes: start 0.75 mg subcutaneously once weekly. After 4 weeks, increase to 1.5 mg weekly when needed; subsequent increases are 1.5 mg after at least 4 weeks at each dose, up to 4.5 mg weekly.

Renal impairment, including ESRD, does not require dose adjustment. Use caution in ESRD and monitor renal function with severe gastrointestinal reactions or volume depletion. The boxed thyroid C-cell-tumor warning and contraindications for personal/family medullary thyroid carcinoma or MEN2 remain applicable. This is not a pediatric dosing card.
