Dosing / renal protocol and limits

HIV single-agent lamivudine: usual 300 mg daily or 150 mg twice daily. CrCl 15-29: 150 mg once then 100 mg daily; 5-14: 150 mg once then 50 mg daily; <5/HD: 50 mg once then 25 mg daily. NIH also lists 100-150 mg daily alternatives at these low clearances with limited evidence. Its 30-49 no-adjustment approach incorporates combination-label guidance, unlike the single-agent Epivir label. Do not transfer to lamivudine/zidovudine.
