Pediatric ARV guidance: lamivudine needs renal adjustment. Do not use lamivudine-containing fixed-dose tablets when CrCl <30 mL/min or on dialysis because individual component doses cannot be adjusted. With sustained CrCl 30-49, the guidance highlights increased exposure and monitoring for hematologic toxicity, potentially requiring a change from the combination.

This is a pediatric-source caution, not a numeric pediatric renal schedule. Adult dose tables elsewhere on this page must not be substituted. Epivir-HBV products have lower concentrations/strengths and are not interchangeable with HIV doses.

HIV and hepatitis-B lamivudine dosing differ. The single-agent and fixed-combination records must not be interchanged.

The preserved source page includes lamivudine and a zidovudine combination. Newly added adult single-agent and pediatric guidance are explicitly scoped; do not transfer single-agent dose changes to the fixed combination. NIH and the Southern African society use different renal approaches and alternatives.

Usual Dosing (Adults)

HIV: lamivudine 150 mg orally every 12 hours or 300 mg daily. Hepatitis B (Epivir-HBV): 100 mg daily. Combivir contains lamivudine 150 mg plus zidovudine 300 mg: one tablet twice daily.
