One-hour labeling versus institutional extended infusion

The RX label table uses a one-hour infusion. The UCSF protocol uses a three-hour infusion. Both also separate urinary/intra-abdominal infection from pneumonia/severe infection.

What to reconcile: Keep the selected source dose, indication, renal band, loading instruction and infusion duration together.

Pediatric renal guidance changed after the RX mirror

The older mirror lacks the age-specific renal regimens now supplied by the May 2026 label.

What to reconcile: Use the new pediatric card for ages two and older; verify indication, pediatric eGFR, weight caps and infusion time. Do not use adult Cockcroft-Gault bands as pediatric instructions.

Dosing / renal protocol and limits

Adults: cUTI/cIAI baseline 1.5 g IV every 8 hours; CrCl 30-50, 750 mg every 8 hours; 15-29, 375 mg every 8 hours; HD, 750 mg load then 150 mg every 8 hours.

HAP/VAP or septic shock: baseline 3 g every 8 hours; 30-50, 1.5 g every 8 hours; 15-29, 750 mg every 8 hours; HD, 2.25 g load then 450 mg every 8 hours. Inpatient infusions run over 4 hours, unlike the label's 1 hour. Only the adult rows are summarized.
