Adult CrCl bands are >=50 / 30-49 / 10-29 / <10 mL/min. For initial orolabial/genital HSV or recurrent-episode treatment with HIV, doses are respectively 1000 / 1000 / 1000 / 500 mg, at intervals of 12 / 12 / 24 / 24 hours. For HSV suppression or the separately listed recurrent-episode regimen, use 500 mg every 12 / 12 / 24 / 24 hours. Primary varicella or herpes zoster: 1000 mg every 8 / 12 / 24 hours, then 500 mg daily in the lowest band.
The intermittent-HD table lists 500 mg orally each evening. Continuous-HD data are insufficient; obtain ID guidance. Select the indication rather than transferring a zoster dose to HSV.
