Renal dosing, restrictions and evidence limits

For adult herpes zoster, the source lists 1 g every eight hours at CrCl >=50, 1 g every twelve hours at 30-49, 1 g every 24 hours at 10-29, and 500 mg every 24 hours below 10. The usual zoster course is seven days. On hemodialysis days, give the recommended dose after dialysis. The source has different rows for cold sores, genital-herpes treatment and suppression; do not transfer this zoster schedule to those indications. Usual Dosing (Adults)

Herpes zoster: 1 g orally three times daily for 7 days. Primary genital herpes: 1 g twice daily for 10 days. Recurrent genital herpes: 500 mg twice daily for 5 days. Chronic genital suppression: 500 mg daily. Herpes labialis: 2 g every 12 hours for 1 day.

For initial orolabial/genital HSV or recurrent-episode treatment with HIV, doses are respectively 1000 / 1000 / 1000 / 500 mg, at intervals of 12 / 12 / 24 / 24 hours. For HSV suppression or the separately listed recurrent-episode regimen, use 500 mg every 12 / 12 / 24 / 24 hours. Primary varicella or herpes zoster: 1000 mg every 8 / 12 / 24 hours, then 500 mg daily in the lowest band.

The intermittent-HD table lists 500 mg orally each evening. Continuous-HD data are insufficient; obtain ID guidance. Select the indication rather than transferring a zoster dose to HSV.
