Dose and kidney adjustment

MAI: 15 mg/kg orally daily; MTB: 15-25 mg/kg daily, in combination regimens. Renal estimate <30 mL/min or intermittent HD: the same indication-specific dose three times weekly, after HD. Use therapeutic drug monitoring.
