Clinical context remains essential
Independent clinical review is pending. Confirm indication, exact formulation/route, population, renal measure and units, kidney-function stability, dialysis prescription, interacting drugs, monitoring and source revision. An RX mirror and its original label are not independent corroboration. Missing disagreement notes do not certify agreement.

EMTRIVA versus NIH: explicit renal schedule deviation
The standalone capsule/solution label reduces dosing at selected bands where NIH uses no adjustment. NIH footnote b explains its incorporation of some fixed-dose-product guidance.
What to reconcile: Choose and document one applicable institutional/specialist approach. Do not average schedules or silently relabel the NIH approach as the standalone product label.

Renal dosing, restrictions and evidence limits
Adult renal table (CrCl in mL/min):
CrCl
Capsule
Oral solution 10 mg/mL
>=50
200 mg every 24 hours
240 mg (24 mL) daily
30 to 49
200 mg every 48 hours
120 mg (12 mL) daily
15 to 29
200 mg every 72 hours
80 mg (8 mL) daily
<15 or hemodialysis
200 mg every 96 hours
60 mg (6 mL) daily
On hemodialysis days, administer after dialysis. Efficacy of these reduced-renal-function schedules has not been clinically evaluated; pediatric renal data are insufficient. NIH guidance differs at selected bands and is stored separately.
