Historical renal / replacement-therapy guidance

At 20-50 mL/min, retain the usual indication dose. At 10-20 and <10, the book recommends an initial 25 mg dose and response-based titration. PD/HD/high-flux use the <10 band; continuous therapy uses 10-20; the drug is described as not dialyzed. This conservative low-function starting approach is distinct from the U.S. label, which ties lower starting doses especially to volume depletion. Monitor potassium and renal function.
