Labeled single-dose cystitis versus repeated institutional dosing
MONUROL labeling is limited to uncomplicated cystitis. UCSF separately supplies repeated oral doses for complicated cystitis and leaves CrCl exactly 50 unassigned between >50 and <50.
What to reconcile: Document the institutional/off-label rationale and exact renal band. Do not use these oral schedules for pyelonephritis or IV CONTEPO.

Renal dosing, restrictions and evidence limits
The labeled regimen is a single 3 g oral dose for uncomplicated cystitis; not pyelonephritis or perinephric abscess. Declining renal function prolonged elimination and reduced urinary recovery. The renal PK paragraph does not establish a repeated-dose schedule for complicated infection. Keep separately attributed institutional off-label schedules distinct from the label.
