Dosing / renal protocol and limits

The regimen is expressed as isavuconazonium: 372 mg IV/orally every 8 hours for six doses, then 372 mg daily. Each 372 mg prodrug dose corresponds to 200 mg isavuconazole.

UCSF retains this schedule without renal or hemodialysis reduction. Check interactions and use antifungal-level consultation when indicated. The prodrug and active-moiety dose numbers must not be interchanged.
