Clinical context remains essential
Independent clinical review is pending. Confirm indication, exact formulation/route, population, renal measure and units, kidney-function stability, dialysis prescription, interacting drugs, monitoring and source revision. An RX mirror and its original label are not independent corroboration. Missing disagreement notes do not certify agreement.
All doses are total piperacillin plus tazobactam. The 30-minute source regimen is not an extended-infusion university regimen. The original PD summary and indication-specific CAPD table differ.

Three-hour and four-hour institutional protocols are not interchangeable
Sinai Health and UCSF differ in infusion duration, initial-to-maintenance timing, renal intervals and dialysis regimens. These are separate local pathways, not duplicate confirmations of one universal dose.
What to reconcile: Choose an approved institution-specific pathway with infection, susceptibility, dialysis settings and IV access documented. Do not pair one source's dose with the other's interval or loading sequence.
The RX short-infusion table separates nosocomial pneumonia and other indications and specifies an HD supplement. UCSF lists a different all-infection intermittent-HD schedule.
What to reconcile: Keep the chosen regimen, infusion duration, indication and dialysis assumptions intact. Do not add the label supplement automatically to an institution's different regimen. Exactly 20 is also unassigned by UCSF's printed greater-than/less-than headings.

Renal dosing, restrictions and evidence limits
For adult inpatients, this policy uses piperacillin/tazobactam 4.5 g with intervals of six hours at CrCl at least 40, eight hours at 20-39, and twelve hours below 20 mL/min. IHD and PD use 4.5 g every twelve hours; CRRT uses 4.5 g every six hours. Infuse the first dose over 30 minutes and subsequent doses over three hours. Start the next dose three hours after the initial dose, except below CrCl 20, IHD or PD, where the next dose starts six hours after the initial dose. Review actual administration times and IV access. This local policy permits exceptions after pharmacy, stewardship or infectious-disease review; it is not an interchangeable version of UCSF's four-hour regimen.
