Dosing / renal protocol and limits

Adult hypertension or diabetic nephropathy: usual starting dose 50 mg orally once daily; increase to 100 mg daily according to blood pressure response. A 25-mg start is recommended with possible intravascular depletion. Renal impairment alone does not require adjustment unless volume depletion is also present. Monitor creatinine and potassium and reassess volume status. The fetal-toxicity warning remains applicable. This adult single-agent statement does not apply to pediatric low-GFR use or a losartan/hydrochlorothiazide combination.
