The introductory adjustment trigger is <50, but the first table band includes 20-50. The dose unit remains indication-specific.

Renal dosing, restrictions and evidence limits

After the usual initial dose, the renal table specifies: CrCl 20-50 mL/min, usual indication-specific unit dose every 24 hours; CrCl <20, one-half of that unit dose every 24 hours. The introductory text says adjustment below 50 but the table includes 50. Resolve that exact boundary before use; do not silently round into a band. A unit dose depends on indication.
