Dosing / renal protocol and limits
Adult heart failure: 3.125 mg orally twice daily for 2 weeks, then, if tolerated, 6.25, 12.5 and 25 mg twice daily at intervals of at least 2 weeks. For hypertension, begin 6.25 mg twice daily and titrate at 1-2-week intervals, to at most 25 mg twice daily.
No fixed CrCl reduction schedule is supplied in this label. During titration, monitor renal function particularly with low blood pressure, ischemic/vascular disease or underlying renal impairment. Reduce or discontinue if renal function worsens. Carvedilol is not significantly cleared by HD. These are immediate-release tablets, not extended-release capsules.
