Usual maintenance dosing versus a lower HF starting dose
The broad renal PK statement permits usual dosing, while the HF section prefers 5 mg initially in moderate/severe renal failure or after vigorous diuresis.
What to reconcile: Match initiation versus maintenance and volume/blood-pressure status. No routine maintenance reduction does not mean no monitoring.

Renal dosing, restrictions and evidence limits
The source permits the usual dose across renal impairment because hepatobiliary clearance partly compensates. However, for heart-failure patients with moderate-to-severe renal failure or vigorous diuresis, it prefers a 5 mg initial dose rather than the usual 10 mg and close blood-pressure observation. Assess renal function and monitor for hypotension, azotemia and hyperkalemia. The no-routine-adjustment statement does not override this initiation qualification.
