Renal dosing, restrictions and evidence limits
The source states that renal impairment and peritoneal dialysis do not require a dosage adjustment. In the pharmacokinetic discussion, peritoneal clearance is negligible; hemodialysis removal is predicted to be unlikely from molecular size rather than established by a dialysis dosing trial. Match the infection-specific regimen and infusion instructions separately. Do not turn a pharmacokinetic prediction into a newly validated hemodialysis protocol.
