Dosing / renal protocol and limits

For adult invasive aspergillosis or mucormycosis, give 372 mg isavuconazonium sulfate IV or orally every 8 hours for 6 doses, then 372 mg once daily. Begin maintenance 12-24 hours after the final loading dose. Each 372-mg prodrug dose equals 200 mg isavuconazole. Infuse IV doses over at least 1 hour, never as a bolus.

No adjustment is needed for mild, moderate or severe renal impairment, including ESRD. Isavuconazole is not readily removed by dialysis. Pediatric regimens and nasogastric preparation instructions require their separate label sections; this card is adult-only.
