Choose one regimen; do not combine them. The label's reduced single dose is 1125 mg, whereas the displayed UCSF page uses 1000 mg. UCSF's two 1500-mg osteomyelitis doses are a different off-label strategy, not this skin-infection label. Pediatric renal dosing below the studied threshold is not established.
