Dosing / renal protocol and limits

Adults generally start 10-20 mg orally once daily; the labeled range is 10-80 mg daily. A 40-mg start may be used when an LDL reduction greater than 45% is needed. Renal impairment does not require dose reduction, but it increases the risk of myopathy/rhabdomyolysis, so monitor symptoms. Hemodialysis is not expected to substantially increase clearance; ESRD was not directly studied in that pharmacokinetic statement. Interaction-based dose limits still apply and are not overridden by the no-renal-adjustment statement.
