Dosing / renal protocol and limits

Single-agent lamivudine, in the society's CKD table: indexed eGFR 30-50 mL/min/1.73 m2, unchanged; 15-30, 150 mg daily; <15, 50 mg daily. HD or PD: first 50 mg, then 25 mg daily; give ART after HD. The adjacent bands overlap at 30; confirm the intended boundary locally.

This non-U.S. professional protocol uses indexed eGFR and differs from the NIH label-dose options at some low-clearance bands. Lamivudine is a U.S.-labeled ingredient, but this is not U.S. FDA dosing. It does not apply to fixed-dose lamivudine/zidovudine.
