Renal regimen by migraine indication

For severe impairment (CrCl 15-29) or ESRD (below 15), the episodic-migraine dose is 10 mg once daily. On intermittent hemodialysis days, administration after dialysis is preferred. For chronic migraine, use is not recommended in severe impairment or ESRD.
