Renal regimen by migraine indication

For severe impairment (CrCl 15-29) or ESRD (below 15), the episodic-migraine dose is 10 mg once daily. On intermittent hemodialysis days, administration after dialysis is preferred. For chronic migraine, use is not recommended in severe impairment or ESRD.

No cross-indication extrapolation

The episodic-migraine renal dose is not an approved chronic-migraine alternative. Drug interactions have additional dose restrictions.

Renal dosing, restrictions and evidence limits

For episodic migraine with CrCl <30 mL/min, including ESRD, use 10 mg orally once daily; preferably administer after intermittent dialysis. For chronic migraine, avoid use in severe impairment or ESRD. No renal modification is recommended for mild or moderate impairment. Severe impairment/ESRD were not studied, so the labeled recommendation and the evidence limitation must both remain visible. Reconcile interaction-based changes separately; the renal rule is indication-dependent.
