Renal dosing, restrictions and evidence limits
Use with extreme caution in renal impairment. Monitor serum concentrations and adjust the dose to prevent accumulation; repeatedly assess renal function, hematologic indices and hepatic function. Obtain baseline electrolytes and hematologic/renal measures.
