Three-hour and four-hour institutional protocols are not interchangeable

Sinai Health and UCSF differ in infusion duration, initial-to-maintenance timing, renal intervals and dialysis regimens. These are separate local pathways, not duplicate confirmations of one universal dose.

What to reconcile: Choose an approved institution-specific pathway with infection, susceptibility, dialysis settings and IV access documented. Do not pair one source's dose with the other's interval or loading sequence.

Label and UCSF dialysis instructions differ

The RX short-infusion table separates nosocomial pneumonia and other indications and specifies an HD supplement. UCSF lists a different all-infection intermittent-HD schedule.

What to reconcile: Keep the chosen regimen, infusion duration, indication and dialysis assumptions intact. Do not add the label supplement automatically to an institution's different regimen. Exactly 20 is also unassigned by UCSF's printed greater-than/less-than headings.

Dosing / renal protocol and limits

For CrCl >20 mL/min, 3.375-4.5 g IV every 8 hours over 4 hours; below 20, 3.375 g every 12 hours over 4 hours. IHD general infection: 2.25 g every 12 hours; severe infection: 3.375 g every 12 hours over 4 hours. CRRT extended infusion: 3.375-4.5 g every 8 hours over 4 hours.
