Dosing / renal protocol and limits
Adult indication
IV dose once daily
Candidemia, disseminated Candida infection, Candida peritonitis or abscess
100 mg
Esophageal candidiasis
150 mg
Candida prophylaxis during hematopoietic stem-cell transplantation
50 mg
Infuse over 1 hour. Renal impairment does not require adjustment, and no supplemental dose is required after hemodialysis. Monitor renal and hepatic tests despite the absence of a dose reduction. The label has not adequately studied Candida endocarditis, osteomyelitis or meningoencephalitis; the institutional high-dose protocols are separate recommendations.
