These products contain zoledronic acid but differ in indications, dosing context and renal eligibility. An oncology reduced dose does not bypass the RECLAST restriction.

Before applying: Confirm the actual ordered product and indication, hydration and renal method. Do not administer both products or transfer the hypercalcemia serum-creatinine criterion to RECLAST.

Renal dosing, restrictions and evidence limits

RECLAST is contraindicated at CrCl below 35 mL/min or with acute renal impairment. At CrCl at least 35, the source does not support a renal dose reduction. Calculate Cockcroft-Gault clearance using actual body weight before each dose. A dose must not exceed 5 mg and infusion must last at least 15 minutes. Withhold during suspected dehydration until volume status is normal and monitor at-risk patients. RECLAST and ZOMETA contain the same active ingredient but have different indications and renal instructions; do not use them together.
