At eGFR >=30 the TRIPLE PAK regimen is vonoprazan 20 mg, amoxicillin 1000 mg and clarithromycin 500 mg, each twice daily. DUAL PAK uses vonoprazan 20 mg twice daily and amoxicillin 1000 mg three times daily. Each is a 14-day regimen. Below 30, use of either co-pack is not recommended. The page also contains single-agent esophageal indications with different renal wording; those instructions must not be substituted for H. pylori co-pack restrictions. Reconcile the exact product and indication before selecting a regimen.
