Renal dosing, restrictions and evidence limits
No renal dose recommendation is established for impaired kidney function. Before treatment obtain cystatin C, urine dipstick and urine protein-to-creatinine ratio, and consider an exogenous-marker GFR. During treatment check urine dipstick monthly and cystatin C/protein ratio every three months. For urine protein testing, use a pre-infusion specimen or one at least 48 hours after infusion, or an assay avoiding pyrogallol red, because excreted drug can interfere. Persistent cystatin-C or protein increases warrant pediatric-nephrology assessment. Low muscle mass in DMD makes serum creatinine an unreliable basis for an invented reduction.
