Indication and usual adult dosing
Original SABRIL indication: adjunctive refractory complex partial seizures after inadequate response to several alternatives, when benefit outweighs vision-loss risk; not first-line treatment.
500 mg tablets orally twice daily with or without food. Start 500 mg twice daily (1 g/day); increase total daily dose by 500 mg weekly according to response. Recommended adult dose 1.5 g twice daily (3 g/day). Six g/day had no additional benefit and more adverse events.
Withdraw gradually. The source adult trials tapered the total daily dose by 1 g at weekly intervals until discontinuation.
Renal Dosing
Primarily renal elimination. Original CrCl bands: above 50-80 mL/min, reduce dose 25%; above 30-50, reduce 50%; above 10 to under 30, reduce 75%. The wording leaves exactly 30 unassigned.
Cockcroft-Gault estimate: (140 - age) x weight in kg / (72 x serum creatinine in mg/dL), multiplied by 0.85 for females.
Hemodialysis
Effect of dialysis on clearance was not adequately studied.
