Dosing / renal protocol and limits

Adult C. difficile-associated diarrhea: 125 mg orally four times daily for 10 days. Adult staphylococcal enterocolitis: 500 mg-2 g/day in 3-4 divided oral doses for 7-10 days.

These oral capsules do not treat systemic infections; an IV vancomycin regimen must not be substituted.

The label supplies no CrCl-based oral adjustment table. Significant systemic absorption can occur with active colitis, especially with renal insufficiency; serum-level monitoring may sometimes be appropriate. Nephrotoxicity has occurred with oral treatment. Monitor renal function, particularly during and after therapy in patients older than 65 years.

Dosing / renal protocol and limits

Initial non-fulminant C. difficile infection: 125 mg orally four times daily. Fulminant disease: 500 mg four times daily under the full severe-CDI pathway; rectal administration may also be considered. The source does not reduce these oral doses for kidney dysfunction or HD.

Oral vancomycin is not a substitute for IV therapy of systemic infection. Taper schedules and additional fulminant-disease treatments require the complete CDI guideline.
